EAR: SMA Box Code?

I received an email looking for a SMA box code to commemorate the passing of a relative who was close to him. 

I have various R&J (petit churchill/short churchill) recorded with an SMA code but not much more that I can find from the records I have on hand here at the moment. 

What other cigars came out of SMA? Please share the cigar and code if you can
