One of the major earners, along with tourism, medic programmes seem to be under the kosh too. This article suggests the medics can stay as independents. Interesting.

Indentured servitude. 

I, the Cuban Govt will supply the doctors for $5000 each a month. You pay me directly. The doctors will receive $2000 each a month. 

It's a great business. About $1.8 Billion great. No need to wonder why they put so much effort into pumping out medical professionals + engineers + teachers etc.

The big payoff is that they can import a car and a container of goodies mostly duty free. They could spend $3000 on a car and once it arrives Cuba its worth $20,000+
