I would assume the cigar or stem of the pipe would rest along the body, and beneath the monkey in the middle to hold it in place.

Wish we had more pictures but it looks like you put your opium, I mean tobacco, in the man’s head and smoke from the feet.

Could use the man's head to pack down the tobacco in the pipe perhaps.
